The Home First service is transforming the way support is delivered to people following discharge from hospital, helping them regain independence and remain safely in their own homes. We are seeking a highly organised and proactive Planner and Administrator to play a vital role in the success of this new countywide service.

Working at the heart of the operational team, you will be responsible for coordinating patient visits, managing service capacity and demand, and ensuring resources are deployed effectively to meet the needs of the people we support. You will also provide comprehensive administrative support to the multidisciplinary team, acting as a key point of contact for staff, patients, carers and partner organisations.

You will be part of a weekend rota, working 1 in every 5, and based at the Home First Hub in Brockworth.

Job description

As a Home First Planner and Administrator, you will be responsible for coordinating and scheduling patient visits to ensure care and support plans are delivered safely, efficiently and in line with individual needs. Working closely with the Home First Leads, Therapy Leads and multidisciplinary team, you will proactively manage service capacity and demand to maximise productivity and support effective patient flow.

You will be responsible for accurately recording and maintaining information on clinical systems, creating and updating schedules, monitoring workforce capacity and responding to changing service demands. You will act as a key point of contact for the service, handling enquiries from patients, carers, staff and partner organisations professionally and efficiently.

The role also includes providing a comprehensive administrative service, including diary management, meeting coordination, minute taking, maintaining records and databases, ordering supplies, supporting invoice processes and ensuring information is recorded accurately and confidentially.

You will work collaboratively with colleagues across Home First, Integrated Community Teams and partner organisations to support safe, effective and responsive service delivery. The successful candidate will be well organised, adaptable and able to prioritise competing demands within a busy operational environment while maintaining high standards of customer service and attention to detail.

This post requires the post holder to be able to undertake all statutory and mandatory training and post holders must be able to undertake physical interventions. Fitness for physical intervention will be assessed by Occupational Health.

Person specification

Qualifications

Essential

• Educated to GCSE level in Mathematics and English to Grade C and above.
• NVQ level 3 in administration or an appropriate equivalent qualification or experience.
• RSA Typing Level 2 or equivalent qualification or experience.

LENGTH AND / OR NATURE OF EXPERIENCE

Essential

• Experience of unsupervised administrative work in a busy office environment
• Experience of working in a customer-facing environment with evidence of telephone communication skills
• Experience supporting teams or managers and setting up/running effective office systems

Desirable

• Previous experience of working with the NHS/local authority

PROFESSIONAL / MANAGERIAL / SPECIALIST KNOWLEDGE

Essential

• Skilled in Microsoft Office Packages, e.g. Word, Excel, and PowerPoint and use of computers and keyboards
• Confident to liaise with other organisations and people as appropriate.

Desirable

• Previous experience and competency of an electronic patient records (e.g. SystmOne)
